This section is designed to help show you the route to obtaining your HGV licence
Starting my new journey
HGV medical (D4 Form)
You will to have a medical by your own GP or an independent doctor and they will fill out a D4 form
provisional HGV entitlement
You will need to complete a D2 form for the HGV, LGV, or PSV provisional entitlement to be applied to your licence
Apply for your hgv provisional entitlement
You will need to send the following documents to the DVLA :- both the completed D2 and D4 forms and your full driving licence.
These must be sent to the DVLA, Swansea, sa99 1bn.
Book my HGV theory test
before you can take your HGV practical on road test you must pass your hgv theory test. you can do this online or we can include this in your training costs.
Book my hgv hazard perception test
before you can take your HGV practical on road test you must pass your hgv Hazard perception test. you can do this online or we can include this in your training costs.
Book my HGV practical training
once you have completed the 5 steps you can start your practical training. Blue Triangle Training Ltd provide practical class 1, class 2 and 7.5t hgv / lgv training course for both the hgv reversing excercise ( module 3a ) test and practical on road ( module 3b ) test.
