Application for lorry provisional licence

Before you can start your HGV training you must have the correct provisional entitlement on your licence, this entitlement is allocated by the Driver and Vehicle Licensing Agency (DVLA). So which HGV entitlement do I need and how do you get this entitlement?
HGV category C1 any ridged HGV vehicle 3500kgs to 7.5t to drive a category C1 vehicle you must apply for C1+E.
HGV category C any ridged HGV vehicle over 3500kgs.
HGV category CE any HGV vehicle with or without a trailer (this is covered by ticking the category C box on the D2 form).
Getting the entitlement on your driving licence you must have a medical examination report D4 form completed by a doctor. this can be done by your own doctor or any approved doctor that supplies this service, there is a charge payable to the doctor used. you must fill out the Application for lorry, bus or minibus driving licence D2 form. We suggest that you put ticks in all the Lorry category boxes and Bus category boxes, this will ensure you have all the Lorry / Bus provisional entitlements, there is no charge for these provisional entitlements to be registered onto you full category B (car) licence. Once you have both the D2 and D4 forms completed send the with your driving licence to the DVLA, Swansea, SA99 1BR. We recommend that you take a copy of your driving licence and send the forms via the proof of delivery service provided by the post office.
